The radial nerve divides just past the elbow into the superficial radial nerve and the posterior interosseous nerve (PIN). The PIN passes through the radial tunnel, where it can become compressed at the arcade of Frohse (the leading edge of the supinator muscle), producing lateral forearm pain without the grip weakness typical of PIN palsy.
Signs and Symptoms
- Aching pain in the outer forearm and lateral elbow
- Pain worsening with resisted forearm supination or wrist extension
- Tenderness approximately 4 to 5 centimeters distal to the lateral epicondyle (distinguishes from tennis elbow tenderness directly at the epicondyle)
- No significant grip weakness in most cases
- Symptoms that persist despite appropriate tennis elbow treatment
Common Causes and Risk Factors
- Repetitive forearm rotation activities
- Fibrous bands or anomalous vessels at the radial tunnel
- Direct forearm compression
- Prior radial head fracture or elbow surgery
